Family information session for Small Supports.

Do you have an autistic young person or adult or young person or adult with learning disabilities living at home or in their own home, but you need to think about the future or get better, more personalised support for you and your loved one?
Small Supports provides a bespoke person-led approach to care and support by matching your loved one with a local Small Support provider who will recruit a team to provide support designed with you and your loved one.
Small Supports are local small social care organisations which can help your loved one to live in their own home, if they:
- are aged 16 and above and need bespoke person-centred support
- have a learning disability and/or autism, or a mental health condition and/or have experienced significant challenges or trauma
- are in a mental health or secure hospital currently or are at risk of hospital admission due to a breakdown of support, lack of the right support, the need to move accommodation, or to independent living
